When things go wrong, vulnerable children, for example, can suffer significant harm, which can result in major disabilities and even death. 

At such times, the legal system will address the issue of investigating who is responsible and whether any criminal proceedings should be instituted. 

For the local authority, the first response will be to begin the process of carrying out a review. 

Whereby they will set up a multi-agency panel to examine social services and other agencies involvement with the family. 



With the objective of trying to find out what went wrong to have resulted in the harm or potential harm resulting from the incident. 

And how it could, or might have been prevented. A more honest interpretation might be to see the brief of the review, as that of finding out who is to be blamed for what happen. 

Who was it who did not - or could not - do their job well enough. 

For the frontline social workers, this is a fearful time.
